What is hospice care?

Hospice provides care to those seeking comfort rather than curative measures. Hospice is designed to treat the patient holistically by focusing on body, mind and spirit, as well as caring for the patient, family and caregivers. Along with your attending physician, the hospice team (physicians, nurses, home health aides, social workers, chaplains, dietitian, and volunteers) works to ensure your final days are meaningful, comfortable, and spent according to your wishes. Hospice also provides medications, medical equipment, medical supplies, and other items or services related to your hospice diagnosis and approved by your physician and the hospice team. Family receives support during and after your care with hospice.

Who qualifies for hospice?

Hospice is for those individuals who seek palliative, comfort care services when there are no aggressive treatment options available, when traditional care has failed, or when the patient decides not to pursue these options. To be eligible, your physician must determine that your disease is in the end stage. A patient may have any diagnosis, not just cancer, to be eligible for hospice care.

What can I expect from your services?

A team of professionals, along with you and your family, will develop a plan of care. Services will be delivered in the patient's residence. The patient will receive care designed to support physical, mental and spiritual well-being. The patient, family and/or caregivers will be supported while they care for their loved one. A nurse and other core team members are available 24 hours a day, 7 days a week. Family will continue to receive support up to 13 months after the patient's death.

Do I need to leave my home to receive hospice services?

Hospice is not a place, or a building, but rather a concept of care. Our team of professionals travels to your home to provide hospice services. You may live in a private residence, assisted living facility or nursing center.

Will Medicare or other insurance pay for hospice care?

Hospice is fully covered by Medicare and most Medicaid programs with no out of pocket costs. Private insurance policies typically provide coverage for hospice care.

How do I access hospice care?

Do I need to have signed a DNR before services can begin?

No. We believe that patients may need support to make this decision. Patients who are eligible will not be turned away if they have not yet decided to sign a DNR. The hospice care team will support the patient in decision-making and in understanding the possible outcomes of these decisions.
